Your First Appointment
Please arrive at least 30 minutes prior to your first appointment. The following should be brought with you to your first appointment.
What to Bring
- Completed New Patient Forms - You may print these forms and complete them before coming into the office. View and download all new patient forms.
- Insurance Card - It’s important to bring your insurance card because health plan benefits and financial obligations vary depending on specific coverage.
- Medication List - Please bring a list of current medications, including dosage information, to every appointment.
- Valid ID - Please bring one form of identification which includes a photo.
- Medical and Immunization Records - If you are a new patient, you will need to complete a medical records release form after your initial visit. Our office will handle the transferring of records.
- Co-payment - Please remember that any applicable co-payment is due at time of service.

Follow-Up Appointments
For future appointments, please arrive at least 15 minutes prior to your appointment. The following should be brought with you to every appointment.
