Joint Notice of Privacy Practices

The Cedars-Sinai entities and affiliates, including Cedars-Sinai Medical Center, Cedars-Sinai Marina del Rey Hospital, Torrance Memorial Medical Center, Huntington Hospital and their respective physician networks and medical staff as described below* (collectively, “we”), are an Organized Healthcare Arrangement and have adopted this Joint NPP, which describes our commitment to your privacy.
This Joint NPP Summary describes how we use and/or disclose your medical information, how you can get access to this information, and what we do to protect your privacy. Please review it carefully.

Uses and Disclosures for Treatment, Payment, Operations. We use your health information to provide you with treatment and to arrange for payment for our services or for healthcare operations. These uses and disclosures are explained in further detail in the full version of the Joint NPP.
Legally Required Disclosures. There are certain situations where the law requires us to share your health information, such as (1) if you are deemed a serious threat to yourself or others in the community, (2) if we are given a legally executed court subpoena from a judge, (3) if a law enforcement official requires us to do so, or (4) for workers’ compensation and similar benefit programs. There are other rare situations like these that are described in the full version of our Joint NPP.
Your Rights With Respect to Your Health Information.
You have the right to:
- Get an electronic or paper copy of your medical record
- Ask us to limit what information we use or share, although we are not required to agree to your request
- Get a list of those with whom we’ve shared your health information
- File a complaint with us by calling 310-517-1165, ext. 71165 if you feel we have violated your rights
- Get a copy of this Joint NPP Summary or the full version of our Joint NPP
- Request confidential communications
- Choose someone to act for you and make choices about your health information
- Ask us to correct incorrect or incomplete information in your medical record
- File a complaint with the U.S. Department of Health and Human Services by calling 877-696-6775
Our Responsibility to Protect Your Privacy. We are required by law to maintain the privacy and security of your protected health information and to promptly let you know if a breach occurs that compromises the privacy or security of your information. We must follow the duties and privacy practices described in our Joint NPP, and we must give you a copy of it. The full version of our Joint NPP provides greater detail about the above uses, disclosures, rights and responsibilities, as well as information about how your health information may be used in our fundraising activities, for research purposes, and to further public health and safety, and may be shared with law enforcement, coroners, medical examiners and funeral directors. We reserve the right to amend this Joint NPP Summary or our Joint NPP, and the changes will apply to all information we have about you. Our Joint NPP is always available online at tmphysiciannetwork.org, upon request in our facilities and offices, or by scanning the QR code below. For your convenience, the full Joint NPP is available in other languages online.
* Cedars-Sinai Medical Center, CFHS Holdings, Inc. d/b/a Cedars-Sinai Marina del Rey Hospital, Cedars-Sinai Medical Care Foundation, Pasadena Hospital Association, Ltd., d/b/a Huntington Hospital, The Huntington Medical Foundation d/b/a Huntington Health Physicians, Torrance Memorial Medical Center, and Torrance Health Association Inc., d/b/a Torrance Memorial Physician Network (collectively, the “Cedars-Sinai ACE entities”), along with the medical staffs of the medical centers/hospitals identified above, and affiliated medical groups, professional corporations, independent physicians and allied health professionals contracting with Cedars-Sinai ACE entities to provide services at Cedars-Sinai facilities.
